Dr. med. Matthias Gründling ist langjähriger Oberarzt in der Intensivmedizin der Klinik und Poliklinik für Anästhesiologie und Intensivmedizin der Universitätsmedizin Greifswald. Wissenschaftlich beschäftigt er sich seit Jahren mit Fragen der Infektionen und der Sepsis beim Intensivpatienten. Im Rahmen des Kompetenznetzwerkes Sepsis (SEPNET) leitet er das Regionalzentrum Greifswald und war an den international anerkannten Studien des Netzwerkes beteiligt. Dr. med. Gründling ist Mitglied der Leitlinienkommission der Deutschen Sepsisgesellschaft e.V. an der Universitätsmedizin Greifswald leitet das Qualitätsmanagementprojekt "Sepsisdialog", das in den vergangenen Jahren zu einer wesentlichen Verbesserung der Behandlungsqualität bei Sepsispatienten am Greifswalder Klinikum geführt hat. Aufgrund der Erfolge gewann die Arbeitsgruppe des SEPSISDIALOG den GLOBAL SEPSIS AWARDS 2017.

Die Fakten sprechen für sich – Sepsis ist weltweit eine der häufigsten Erkrankungen und die Haupttodesursache bei Infektionserkrankungen mit einer Sterblichkeit von 30 bis 60 Prozent – oft weil sie nicht früh genug erkannt wird.
 
Viele Patienten kommen mit infektionsähnlichen oder sehr unspezifischen Symptomen in die Notaufnahme, manche entwickeln eine Infektion nach einer OP oder einem schweren Trauma.Besonders bakterielle Infektionen können sich auf die Blutbahn ausdehnen, eine systemische Infektion und eine außer Kontrolle geratene Immunreaktion verursachen, die wir als Sepsis bezeichnen. Dieser ernste, manchmal sogar lebensbedrohliche Zustand ist für den Tod von mehr Patienten verantwortlich als AIDS, Prostata- und Brustkrebs zusammen.

Eine Sepsis ist oft schwer zu erkennen, insbesondere im Anfangsstadium und stellt für Notaufnahmen und Intensivstationen eine erhebliche diagnostische Herausforderung dar. Bleibt eine Sepsis zu lange unerkannt oder wird nicht schnell genug eine effektive Antibiotikatherapie und Kreislaufstabilisierung eingeleitet, kann die Erkrankung rapide zu zusätzlichen Organversagen oder sogar einem septischen Schock führen.

Die Letalität von  Sepsis und septischem Schock kann bei bis zu 50% liegen; bei einer Verzögerung der Antibiotikatherapie und Herdsanierung steigt die Sterblichkeit rapide an.

In diesem Webinar beleuchtet Dr. med. Matthias Gründling die verschiedenen Aspekte die zur Diagnostik und Therapie des Notfalles Sepsis besonders zu beachten sind.
